PACE (Program of All-Inclusive Care for the Elderly) was developed to answer the many problems around caring for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.
Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to an On Lok PACE center for primary medical care, social and recreational activities, and other senior care services.

POSITION SUMMARY: Under the administrative direction of the Program Manager and in consultation and supervision with the Dietitian Specialist, the Dietitian will coordinate the dietary services of the Day Health Center and offer direct services to On Lok participants and allied health professional team members.
DUTIES / RESPONSIBILITIES:
1. Visits and observes participants in order to evaluate their nutritional needs. Reviews diet orders in charts and confers with physicians, nurses, and participants' family, when necessary. Completes dietary assessments and quarterly progress notes for participants as required.
2. Works with and participates actively in the interdisciplinary team regarding diet and nutritional problems and management, including participant care plans, problem solving and case review. In collaboration with the Center/Program manager and other specialists, promotes staff development in the area of nutrition.
3. Provides nutritional education to participants, family and staff as needed; coordinates use of community resources. Provides information to the Nutrition Services Manager concerning preparation of meals for participants of the Day Health Center including menu planning, diet textures, and nutritional planning.
4. Fulfills all requirements, such as reporting, keeping statistical records, making entries into charts and attending meetings as required.
5. Other related duties as assigned.
QUALIFICATIONS (knowledge, skills, abilities):
1. Graduate of an approved Dietetic program.
2. Registered by the Commission on Dietetic Registration.
3. Minimum of two years as a Registered Dietitian in a clinical setting with experience working with the frail, elderly or geriatric population required.
4. Ability to work independently, as well as with the interdisciplinary team.
5. Experience in leadership and training.
6. Flexible, respectful, resourceful, self-motivated, and professional.
7. Bilingual ability is preferred for specific assignments.
8. California Driver's License and willing to drive and or travel for participant visitation (preferred).

Your final compensation offer will be determined based on factors such as skills, experience, education, and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.
