PACE (Program of All-Inclusive Care for the Elderly) was developed to answer the many problems around caring for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.
Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care, social and recreational activities, and other senior care services.

DUTIES / RESPONSIBILITIES:
- On behalf of Lifeways' participants, elicit and identify need for Federal and State entitlement programs and public
assistance programs such as Medicare, Medi-Cal, SSI, SSA, CAPI, General Assistance, etc. - Understand and act on situations where there may be changes in status to participants' eligibility for entitlement
or public assistance programs. - Resolve discrepancies or other issues related to a participant's public assistance program eligibility as needed
including Medi-Cal, SSI, SSA, CAPI and Medicare. - Ensure on-going Medi-Cal eligibility for On Lok Lifeways participants, through ensuring timely and accurate
redetermination applications, as well as identify and apply for those that would qualify for Medi-Cal benefits. - Develop key relationships with county social services department in all Lifeways' service areas.
- Actively work with Marketing & Enrollment (M&E) department to assist the staff in determining a potential
participant's Medi-Cal status and potential share of cost/premium. - Understand and apply rules, regulations, and policies related to entitlement and public assistance programs and
work collaboratively to address participant status issues with Lifeways staff such as Social Workers, M&E staff,
Finance staff, etc. Serve as a resource to staff and provide in-services as needed. - Train or in-service Program Operations and M&E staff as needed and develop training materials as necessary.
- Develop and apply policies and procedures governing Lifeways' RCFE payment.
- Identify available resources, and communicate with others to obtain and verify information concerning eligibility.
- Other duties as assigned.
QUALIFICATIONS (knowledge, skills, abilities):
- Bachelor's Degree or 3+ years' experience determining eligibility for California's entitlement and public assistance
programs - Excellent interpersonal, written and verbal communication skills
- Ability to explain and assist with complicated and detailed financial situations involving entitlement and public
assistance programs. - Excellent time management skills, ability to work independently and multi-task
- Demonstrates good judgment, resourcefulness, flexibility and problem solving
- Possesses sensitivity and awareness of diverse cultural perspectives and the ability to work with diverse
populations - Proficiency with MS Office (Word, Excel, and PowerPoint) required
- Valid CA Driver's license in good standing with personal automobile and insurance
- Ability to work non-standard hours when needed
- Bilingual in Spanish/English or Cantonese/English skills are preferred.

Your final compensation offer will be determined based on factors such as skills, experience, education, and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.
