Dedicated to The Care of Elders:

PACE (Program of All-Inclusive Care for the Elderly) was developed to answer the many problems around caring for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.

Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care, social and recreational activities, and other senior care services.

POSITION SUMMARY: Under the direction and supervision of the Program or Center Manager, the Activity Aide implements activity programs for participants.


DUTIES / RESPONSIBILITIES:

  1. Assists with implementing and participating in group and individual therapeutic activities as well as offer creative modifications (oriented towards group, individual and special needs participants).
  2. Socializes with participants.
  3. Actively participates in team discussions; offers feedback, ideas and suggestions both on a day-to-day basis and in department staff meetings.
  4. Keeps Activity Therapist informed of needed supplies and equipment.
  5. May be responsible for maintaining neatness and order in the indoor/outdoor programs space.
  6. May monitor and report changes in mood & behavior of the participants to the Activity Therapist and Center Manager.
  7. May assist with personal care of participants as needed; for example, escorting the participant to the restroom and or toileting, and/or assistance with feeding.
  8. Other related duties as assigned.

QUALIFICATIONS (knowledge, skills, abilities):

  1. Interest in working with the elderly and understanding of the psychosocial problems of aging.
  2. One year experience working with the elderly or frail population required.
  3. May require current certification in CPR and First Aid.
  4. Ability to work as part of multidisciplinary team.
  5. Stability, maturity, creativity and flexibility in work.
  6. Ability to work with volunteers and contract staff.
  7. Fluency in a second language helpful (Cantonese or Spanish) and written and spoken fluency in English required.

Below are addtional qualifications if your assignment is based out of the Montgomery Center:

QUALIFICATIONS (knowledge, skills, abilities)

  1. Bilingual (Cantonese and English) required.
  2. Experience working with seniors with dementia required.

Your final compensation offer will be determined based on factors such as skills, experience, education, and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.
