PACE (Program of All-Inclusive Care for the Elderly) was developed to answer the many problems around caring for frail seniors. The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.

Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care, social and recreational activities, and other senior care services.

Under the direction and supervision of the Quality Manager, the QA Assistant supports the Quality department, conducts site reviews to monitor the quality of services provided to the frail elderly, and conducts contact investigations.


  1. Assists in preparing credential files for review by the Credentials sub-committee at the scheduled quarterly meetings.
    1. Processes applications of new providers as they are received and reappointment applications of existing providers every 3 years.
    2. Requests expiring documents from providers on a monthly basis to ensure all fires comply with the state and federal regulations.
  2. Conducts site visits, summarizes findings of On Lok operated centers, the Institute on Aging and contacted entities, and shared appropriate findings to the varying committees. The findings include:
    1. Contracted specialist offices and skilled nursing facilities.
    2. Dental clinics operated by On Lok and the Institute on Aging.
    3. Infection Prevention Devices at medical clinics operated by On Lok and the Institute on Aging.
    4. Medical clinics operated by On Lok and the Institute on Aging.
    5. Adult day health centers operated by On Lok and the Institute on Aging.
    6. Program related housing operated by On Lok.
  3. Conducts participant and staff contact investigations and contact tracing for infectious diseases such as COVID-19.
  4. Coordinates, participates, monitors and prepares for various QA Committees including but not limited to:
    1. Quality Management Team
    2. Quality Assurance and Improvement Committee
    3. Dental Review Committee
    4. Infection Control Committee
    5. Home Health Care quality of care review
  5. Writes meeting minutes for quarterly committees, drafts meeting agenda for committees and creates or disseminates quarterly QA newsletter.
  6. All other duties as assigned.

QUALIFICATIONS (knowledge, skills, abilities):

  1. Bachelor's degree from an accredited college or university.
  2. Minimum 1 year of experience in the Healthcare field, preferably medical office..
  3. Ability and means to travel as needed, proof of liability and property damage insurance on vehicle used is required.
  4. Excellent written and verbal communication skills.
  5. Excellent organizational skills with attention to detail.
  6. Proficiency in Microsoft Suite, including Word, Excel, and Outlook.
  7. Proficiency in verbal Cantonese or Spanish preferred.
  8. Availability on weekends and after hours as needed.
  9. Completion of Contact Investigation training by 3 months from hire.

Your final compensation offer will be determined based on factors such as skills, experience, education, and licenses/certifications. The pay range is expressed as an hourly rate regardless of FLSA status. Exempt positions are paid salaried and are not subject to overtime.
